How does the health exam work now because of COVID-19? I understand to get the work permit you need to get a health checkup where you are, and then submit it to the Taiwan office. How does this work? Is it submitted electronically?

Correct. The Taiwan Consulate must approve the health check you get abroad. After that, you send it to your employer and they use it to apply for your ARC.
There is a pdf form you can fill out provided by immigration for health checks , but that’s not mandatory.
I recommend contacting the Taiwan consulate nearest you first.
